Up coming Vacation * August 13-23 Closed for the day no grooming or Day Stay, boarding is available during this time *** ALL PETS COMING IN FOR BOARDING OR GROOMING MUST HAVE THE FOLLOWING VACCINATION DOCUMENTATION SHOWING THAT THE VACCINATIONS MUST HAVN BEEN GIVEN BY A LICENSED VETERINARIAN. 1) RABIES CERTIFICATE (SIGNED BY VET) 2) DISTEMPER PARVO 3) BORDETELLA (KENNEL COUGH) Please note that the vaccine does not guarantee full protection as this is like the flu and common cold in humans there are several strains and pathogens that it is made of. 4) UP TO DATE NEGATIVE FECAL (DONE WITHIN 6 MONTHS OF REQUESTED BOARDING DATES) THE REPORT MUST SHOW NO INTESTINAL PARASITES (WE REQUIRE A NEW FECAL EVERY 6 MONTHS) ***NOTICE Injury's can occur during boarding. also Injury can occur if you are boarding individually or multiple pets in a single run. If you choose to board multiple pets in the same run you the pet owner are assuming all responsibility (both medically and financially) of any injuries that could happen. And you will not hold Pet Haven liable as such. ILLNESS/INJURY : If the pet becomes injured/ill, you the owner shall be notified. You the owner can give Pet Haven permission to call a local veterinarian, or you can call your veterinarian yourself for treatment. You the owner accept all financial responsibility for pet care, will pay for treatment to veterinarian office, and will not hold Pet Haven Liable. PET CARE: Pet Haven can not be held liability for any injury sustained by/to your pet while in our care. All pets that are boarded and otherwise cared for by Pet Haven. Pet Haven will not held liability for injury to pet, loss or damaged from disease, death, running away, theft, fire, injury to person, other pets, or property, or other unavoidable causes.
